چکیده

OBJECTIVE To understand the cause of intermediate ePTFE graft failure when a venous boot is used. This might give insight into the improvement in patency that results from the use of a venous boot. DESIGN A histological and radiological study of failed and failing grafts. METHODS A review of patients operated upon in St Mary's Hospital under the care of a single consultant. RESULTS Four anastomoses were retrieved following irretrievable occlusion and found to have myointimal hyperplasia at the function between the ePTFE graft and collar. The artery was relatively spared. In a further six patients these results have been corroborated by angiography. CONCLUSIONS Myointimal hyperplasia occurs at the interface between ePTFE and vein collar and the recipient artery is relatively spared. This manipulation of the site of myointimal hyperplasia may result in improved patency rates.

Anastomotic Hemodynamics at the Distal End of Lower Extremity Bypass to Improve Patency

rosthetic arterial grafts constructed from expanded polytetrafluoroethylene (ePTFE) have been used for many years as an alternative to autologous vein grafts for lower extremity bypass. When anastomosed above the knee, their performance is similar to that of autologous vein for up to 2 years after operation, thereafter they are associated with a significantly higher failure rate.1 When anastomo...

In vivo cell seeding with anti-CD34 antibodies successfully accelerates endothelialization but stimulates intimal hyperplasia in porcine arteriovenous expanded polytetrafluoroethylene grafts.

BACKGROUND The patency of AV expanded polytetrafluoroethylene (ePTFE) grafts for hemodialysis is impaired by intimal hyperplasia (IH) at the venous outflow tract. The absence of a functional endothelial monolayer on the prosthetic grafts is an important stimulus for IH. In the present study, we evaluated the feasibility of capturing endothelial progenitor cells in vivo using anti-CD34 antibodie...

Paclitaxel-coated expanded polytetrafluoroethylene haemodialysis grafts inhibit neointimal hyperplasia in porcine model of graft stenosis.

BACKGROUND The main pathology of haemodialysis graft stenosis is venous neointimal hyperplasia at graft-venous anastomoses. Neointimal hyperplasia is also observed in cases of coronary artery in-stent restenosis. Paclitaxel is a chemotherapeutic agent used to treat cancer, and has been proven to inhibit neointimal hyperplasia of coronary artery in-stent restenosis. In this study, we examined wh...

Preliminary comparative study of small amplitude helical and conventional ePTFE arteriovenous shunts in pigs.

Intimal hyperplasia (IH), which causes occlusion of arterial bypass grafts and arteriovenous (A-V) shunts, develops preferentially in low wall shear, or stagnation, regions. Arterial geometry is commonly three-dimensional, generating swirling flows, the characteristics of which include in-plane mixing and inhibition of stagnation. Clinical arterial bypass grafts are commonly two-dimensional, fa...

Expression of C-reactive protein in myointimal hyperplasia in a porcine arteriovenous graft model.

BACKGROUND The migration and proliferation of myofibroblasts are prominent features of myointimal hyperplasia associated with haemodialysis polytetrafluoroethylene (PTFE) grafts. Since C-reactive protein (CRP) possesses functional activities on vascular smooth muscle cells (SMCs), we examined the expression of this protein in PTFE grafts early in the course of myointimal hyperplasia development...
